I/We declare that the information in this application is complete and accurate to the best of my/our knowledge.

On behalf of the organisation named in this application, I/we apply for Affiliate membership of Hotel Association Zanzibar (HAZ). If approved, I/we agree to:

  1. abide by the HAZ Code of Conduct, membership requirements, rules and regulations in force from time to time;
  2. pay the applicable annual membership fee;
  3. keep the organisation in good legal and regulatory standing;
  4. notify HAZ promptly of any material change to the information in this application, including changes in ownership, directors, partners, principal management or business activities;
  5. conduct our business in line with the values and professional standards of HAZ;
  6. support the objectives of HAZ and contribute constructively to the development of Zanzibar's hospitality and tourism sector.

I/We understand that membership is subject to approval by the HAZ Membership / Executive Committee, and that submitting this application does not by itself constitute acceptance.
